NEVADA CONSUMER HEALTH DATA PRIVACY POLICY
CE Kitchen Inc (dba Clean Eatz Kitchen)

This policy is provided under Nevada Revised Statutes Chapter 603A and describes how CE Kitchen Inc ("we," "us," or "our") collects, uses, and shares consumer health data of Nevada consumers.
Washington residents should instead refer to our Consumer Health Data Privacy Policy, provided under the Washington My Health My Data Act.
1. Consumer Health Data We Collect, and Why
Dietary restrictions, preferences, and allergies you provide to us. We use this to fulfill your orders accurately, to answer your questions about ingredients and allergens, and to recommend products that match what you have told us.
Health goals and nutritional objectives you provide to us, such as weight management or nutritional targets. We use this to recommend products and to respond to your inquiries.
Meal plan and product selections that indicate a health-related purpose, including your selection of plans marketed for a particular nutritional or health objective. We use this to fulfill your orders and to provide customer service.
Inferences about health status that we may draw from the products you browse or purchase. We use these to recommend products and to plan our menu.
Benefit program participation, for customers who receive our meals through a health plan or government benefit program, including eligibility periods and authorized meal quantities. We use this to confirm eligibility, fulfill authorized orders, and obtain payment from the sponsoring program.
We do not use consumer health data to target advertising to you or to build advertising audiences.
2. Sources of Consumer Health Data
- Directly from you — through account creation, checkout, forms and questionnaires, order notes, product reviews, survey responses, and communications with our customer service team.
- Automatically — from your interactions with our website and mobile application, including pages and products you view, from which we may draw the inferences described above.
- From health plans and benefit program administrators — eligibility and authorization information for customers enrolled in those programs.
3. Consumer Health Data We Share
- Dietary restrictions, preferences, and allergies
- Health goals and nutritional objectives
- Meal plan and product selections that indicate a health-related purpose
- Benefit program participation, eligibility periods, and authorized meal quantities
We do not sell consumer health data.
4. Third Parties and Affiliates With Whom We Share Consumer Health Data
Categories of third parties:
- E-commerce and order management platforms — Shopify Inc.
- Email and SMS messaging platforms — Klaviyo
- Mobile application platforms — Vajro
- Customer reviews and customer service platforms — Stamped.io, Okendo
- Health plans and benefit program administrators — for customers enrolled in such programs
- Legal and regulatory recipients — where required by law, legal process, or governmental request, or where necessary to investigate a food safety issue or product recall
- A successor entity — in connection with a merger, acquisition, bankruptcy, or sale of assets
Specific affiliates: We do not share consumer health data with any affiliate.
We do not share consumer health data with advertising networks, advertising platforms, data brokers, or social media platforms.
5. Collection of Consumer Health Data by Third Parties Over Time and Across Services
No third party collects consumer health data through our website or mobile application over time and across different internet websites or online services.
We use third-party advertising and analytics technologies on our website that collect information about your activity over time and across other websites. We do not permit these technologies to collect consumer health data as described in Section 1, and we exclude consumer health data from the information transmitted to advertising platforms.
6. Your Rights and How to Exercise Them
You have the right to:
- Confirm whether we are collecting, sharing, or selling your consumer health data, and to review that data;
- Know the third parties and affiliates with whom we have shared or to whom we have sold your consumer health data;
- Withdraw your consent to our collection of your consumer health data and, separately, to our sharing of it;
- Request deletion of your consumer health data. On a verified request we will delete it from our records, including archived and backup systems, and notify all processors and third parties that received it to delete it as well; and
- Not be discriminated against for exercising any of these rights.
To submit a request: email support@cleaneatzkitchen.com with the subject line "Nevada Consumer Health Data Request," stating which right you are exercising and the email address associated with your account. You may also write to CE Kitchen Inc, Attn: Privacy, 306 Old Dairy Rd, Wilmington, NC 28405. You may designate an authorized agent to submit a request on your behalf.
We will verify your identity before responding, and will respond within forty-five (45) days. Where reasonably necessary we may extend that period by an additional forty-five (45) days and will notify you of the extension and the reason within the initial period.
If we decline your request, we will tell you why, and you may appeal by emailing support@cleaneatzkitchen.com with the subject line "Nevada Consumer Health Data Appeal." You may also submit a complaint to the Nevada Attorney General's Bureau of Consumer Protection at ag.nv.gov.
